Have you taken advantage of the Physical Exam reward? This is now an annual reward!
Effective July 1, 2011, a $25 reward will be paid for every enrollee who gets a physical exam from their primary care physician. If you have a single plan, you receive $25. If you have a family plan, up to two family members per plan year can submit for the reward for a total reward of $50 ($25 each). The same two or two other family members, up to two per plan year, are eligible for the reward in future years. The Physical Exam Reward Forms are found under the ‘Forms’ tab on the Web site.
