We believe one of the best ways to achieve good health and reduce health care costs is to empower individuals to take personal responsibility for their health. That's why we have developed a wellness program that offers two key incentives: annual reimbursements for various fitness activities and an annual reward for visiting your primary care physician for a physical exam
Here is a sampling of what the Trust can offer you:
The NY44 Living Healthy Rewards Program.
The Fitness Activity reimbursement pays you to stay healthy by joining a health club or buying fitness equipment for your home. Likewise, the Trust pays districts when they provide health education materials at work. Learn more.
The Physical Exam reward offers a $25 payment when you get an annual exam from your primary care physician. Effective July 1, 2012 this reward can be earned annually. Learn more.
